How to Refer:

1. Complete our Provider Referral Form or have the client contact us directly.

2. Once we receive the inquiry, we will reach out to the client to coordinate scheduling and services

If submitting an inquiry on a client's behalf, please ensure:

• The client has authorized Mind Mosaic to contact them regarding this referral.

• The client is aware that we are a private-pay practice.

• We highly encourage providing a Release of Information (ROI) alongside the referral to allow for seamless clinical collaboration.
